| The Real Eat The Real Eat is a specialist dietetics service providing personalised nutrition support to individuals with disability and complex nutrition needs. Their dietitians have expertise in NDIS, tube feeding, blended feeding and feeding differences, taking a practical and person-centred approach to nutrition. The Real Eat supports individuals and families to develop safe, sustainable and positive relationships with food while working towards meaningful health and wellbeing goals. | Occupation Therapy At Home OTAH is a community-based Occupational Therapy service supporting adults living with disability to build independence and participate more fully in everyday activities that are meaningful and necessary to their lives. OTAH takes a person-centred approach, recognising the importance of purposeful engagement, authentic relationships and supportive environments. They work with participants aged 17 years and over across South Eastern Melbourne, including those who are self-managed or plan-managed. |
